Lamictal Crash in pm

My son has been on Lamictal for over 2 years at a low dose (25mg).  He was on other meds prior as well but currently is only taking the lamictal.  I've noticed that it seems he has a "crash" in the afternoon where he gets very emotionally reactive, cries easily, irritable, etc., and then within a short time bounces in the other direction and gets hypomanic, with implusive and silly behavior.  I'm wondering if this is what happens as the lamictal is wearing off?  Should we consider a second small dose in the afternoon to prevent this?

Hi Julie,

 My dd and I both take Lamictal. From my understanding, There is a theraputic dose and that usually is around 200 mgs. Now I know this could be different for many people. It sounds like he is having a hard time with his moods in general. My dd and myself split the doses morning and night. For me... I have missed my morning dose 2 times and not knowing I have missed it and I feel absolutly sick and weird by around mid night time. So, I would definitely talk to his pdoc and explain his struggles and see what they say. Good luck to you. Hopefully you will get this figured out soon.

I don't really think this is due to the Lamictal wearing off.  That is a very, very small dose of Lamictal, and it could be that as he has gotten older he simply needs a higher dose for stability.   What you are describing reminds me of my ds15 when he was younger.  We added lithium at that time and it really smoothed out the evening rollercoaster.

Thanks. I'm charting his moods and we're going back to pdoc in 2 weeks.  We upped the lamictal to 37.5mg now.  It seems like he's doing ok at school and if everything is running on schedule and smoothly...but if something unexpected comes up or his schedule is upset it sets him off in a bad way.
