Can Invega replace Risperdal?

Daughter is bad with mania and psychosis.  Taken off Risperdal Dec. 5.  No AP since then.  Just starting Invega so she will agree to take it knowing that it does not cause the weight gain that her risperdal caused.  Scared that it won't do the same job.  She needs help fast to pull her back from the cliff she is hanging over right now!  Lithium is at 900 mg. but levels are only at .6.  New pdoc said she was satisfied with that?  No other meds in the mix now, as celexa was just stopped.

My daughter started Invega Sustenna a month ago (substained release Invega). Since starting she has gained 10 lbs. Her cholesterol has increase from 189 to 215. I am not sure the increase in cholesterol was due to the Invega Sustenna as there was 4 months between cholesterol tests. Her pdoc added Topamax to decrease my daughter's appitite. I am hoping that the ongoing dose will not cause as severe of increase in appitite as the starting doses did. The ongoing dose is 39mg while the starting doses of 156mg followed a week later by a dose of 117mg.

Most clinical experience seems to show that Invega is not quite as potent as Risperdal.  Generally speaking it does not have as many SE's, including wt. gain,, at 3mg or 6mg, but the SE's increase when you get to 9mg (a high dose).  Given the severity of your dd's sx Pilgrim, I suspect she will need the higher dose.   She may indeed have wt gain, but it could be worth it if it controls her psychosis.
